Please note: downloading a form does not complete the application. The signed form and all supporting documents must also be submitted through the online application portal.

Complete the learner and parent or guardian information, then upload your signed form and required documents.

Both parts must be completed before the Mindscape Education team can begin processing your application.
Complete and sign the form
Download the form for the correct grade group. Fill it in completely and ensure that all required signatures are included.
Complete the online submission
Enter the required learner and parent or guardian information, then upload the signed form and every supporting document.
Prepare before you start
Documents you will need
Have clear digital copies of the following documents ready before opening the application portal.
Parent or guardian’s identity document
Account holder’s identity document
Learner’s identity document or birth certificate
Most recent progression report showing grade completion
Learner’s concession approval, if applicable
Completed and signed Mindscape Education registration form
Grade 11 and Grade 12 applications: please also submit the learner’s Grade 10 and Grade 11 reports.
